If you already have your National Provider Identifier (NPI), but have recently changed your office contact number, address or contact person, you will need to update your NPI as soon as possible. Depending on whether you qualify as an individual or small group practice, or are considered a large organization, your qualification will determine how you can update your registration NPI.

Determine if you are considered a large organization by NPI Enumerator, Inc.If your practice (solo or group) was joined together with other groups for the purpose of obtaining ISFL lots, then you are a large organization and entitled to use the Electronic File Interface (EFI) to update the numbers assigned to your organization. An example of a large organization would be several individual practices coming together to offer services for the Department of Health and Human Services, such a group would be ISFL issues under a single organization rather than individually to facilitate claims tracking and processing Health and Human Services. If you qualify, use the “Electronic Exchange Summary File” at the bottom of this article for more information about the registry to use the EFI service. If you don’t qualify as a great organization, read on. Select whether to apply to update your NPI online or by mailing a paper application. Submitting an application online tends to be faster. If you want to submit a paper application, go to the NPI main site download and print form CMS 10114. If you want to submit documentation online, go to the NPI site using the link at the bottom of this article. Either way, the information needed to fill out the forms will be the same. Fill in the form with the following details: legacy provider number (this is your original NPI number), UPIN number (this is the assigned access PIN number for the NPI administrative network), taxonomy code (these are your special codes for healthcare), the license numbers of all professionals participating in your group, the group’s postal address, the physical address of the practice, and the name and telephone number of the contact person. You will also be asked to provide your name, social security number, date of birth, and license number (of anyone) in the section detailing who you applied.
